Helix Issue With Eshop UPS Shipping

Hi i am using Helix template framework and eshop as my ecommerce component. I have some template issues as you can see in the screenshot when i want to go to step 4 to choose the shipping method. I have set a UPS plugin for my shipping method and everything is set properly. But i get error in the front end as you can see in the screenshot. I have also attached the error

Can you please advice ?

Thanks for contacting us. Sorry for the inconvenience. Will you please capture a screencast video when you finds the error. Besides, Provide me the Joomla administrator access via PM. Does it works with the Joomla protostar Template?
