ad 1) As always in Joomla, you have to create a separate translation for each form.
Support > Documentation > SP Page Builder > How To Tip > How to translate contact form...

ad 2) Since last year, Google Maps API request Credit Card (Money, Money...)
Support > Documentation > SP Page Builder > FAQ > Google maps is grayed
Tip: Use Open Streets Maps addon instead or pay for API

ad 3) Module "Quick Link" wasn't assigned on that page (probably) - double check

The last question - is because suddenly it is a bottom with. “Waves” from one of the other template??

"Waves" cannot migrate alone from template to template, they are related with single template or SPPB Row options.

In this particle case, "wave" (curve) shape is from template styles

.down-curve-white::after {
content: '';
position: absolute;
top: auto;
bottom: -43px;
background: url(../images/down-white.png) no-repeat scroll center center / cover;
height: 43px;
width: 100%;

To remove it, inside template custom CSS just add/use

.down-curve-white::after { display: none; background: transparent;}

I just took one of the PB-pages "contact us" and gave it standard template - but obviously it was not enough
