Hi,
after update PB to 3.1.2 vewrsion, the contact form visually it is wrong.
Please, look http://www.pannivita.com/ in "contacto" menu.

I have Joomla 3.8.5 and qubic template.

try this :

In contact addon file (site. php) probably inside template folder there is mistake
