the only basic way is ... use Form Builder addon instead - it was presented in SPPB 3.4.x update.
That addon allows you to add new custom fields, change the size of them, order etc.

More info here:
I built the form but it doesn't send and doens't give the success message entered. Any ideas?

In most cases:
1) Messages go to the SPAM folder
2) Wrong settings
3) The server doesn't let PHPMail out

As a simple test, please use Joomla Contact component and try
Well i know the website/server is sending mail since the registration e-mails are sending fine.
After submitting the form, the ajax spinner shows momentarily and the contact form fields clear but there is no confirmation the e-mail is sent, you just end up with a blank form.
No errors either
I realise it's when google recapture is enabled on the form, the form doesn't send.
Disabling recaptcha and the confirmation message shows and the mail is sent.

In that case, It may be related to used recaptcha type.
In new Joomla you can choose two main CAPTCHA types (reCAPTCHA and Invisible reCAPTCHA), which one you used?
If I may suggest... try with reCAPTCHA v2 first, and keep "Invisible reCAPTCHA" disabled.

Make sure that keys are Okay for v2.
