Formbuilder Refuses To Send, All Settings Look Correct...?

Hi..

I've been trying to get Formbuilder working and am now running into a failed send. I can use the regular form with same captcha (invisible recaptcha) with success. What's the key to getting to work?

I was able to resolve the issue but turning off required on all fields. The next form sent fine. I will see if I can get it to accept required fields later.
