sendmail_path = "/usr/sbin/sendmail -t -i -f [email protected]"
sendmail_from = [email protected]
adding the above entries to php.ini solves the problem

Czesc Marek,
Since this is a mandatory field, without it you can't send a message, so I suggests entering the full RODO formula in the body of the message (Email Body).
BTW
probably you forgot to put field name tag " {oswiadczenie} "

To zależy jak dodałeś to pole zgody. Jeśli jako dodatkowe pole w forumularzu, to musisz je niestety ręcznie wkleić do treści wiadomości, tak jak pozostałe. Ale lepiej jest użyć klasycznej metody dla tej zgody. Bowiem liczy się efekt końcowy.

Only from Global Configuration, our addon doesn't have Server Maling settings like Joomla or Acymaling.

Jeśli nie ma w opcjach, to nie. Ale o wiem w ConvertForms także nie ma jakiś wyszukanych opcji do wysyłki. Wszyscy opierają się na niezbędnym minium, jedynie AcyMaling oferuje pełne spektrum możliwości.
