Contact Form generating PHP warnings

Version 6.7.0 of SP Page Builder is generating the following PHP warnings from the Contact Form (this issue has also been in previous versions):

I've got this issue across three different sites. This is in the pagebuilder contact form, what is defined as the additional_header & from?

Sorry, I have security to block logins from outside the US. I saw an attempt to login from Poland & Bangladesh, which country are you logging in from?

The issue are these PHP warnings showing up in the website.php.error.log:

Have you used Form Builder on an SP Page Builder page? Then check the addon settings. Hopefully, the error will be gone.

Yes, I have used the Form Builder addon, that isn't the issue. I am using the Contact Form addon and it has this issue. I prefer the Contact Form addon because it has a few simple fields. The solution you're recommending is to simply not use the Contact Form addon?

I have adjusted the padding and margin settings. There was inconsistent values like 20 and 20px together. I have also checked other settings on the addon.
