I saw that you have Securitycheck Pro component.

In most cases missing icons are caused by extra software / htaccess rule that locks loading some SPPB files.

OK, I found the reason.

.htaccess that you had, generated by Securitycheck Pro locked those icons.
Now when I set default .htaccess file from Joomla core, icons are OK, fixed.
