Page For Car Pooling

We would like to add a page on our website for car pooling Just simple, a client put the date he will arrive, from where etc and his email address. Others can read this on our site and react via the email the one above wrote down.
Ones a month i will clean this page. Does somebody has experience with this and wich andon to use? I have page builder pro and helix template.

If Form Builder addon to build a form is not enough, you have to use different booking component.
Or ask on offcial forum.joomla.org or FB Joomla community maybe sobody have already made solution that can share with you.
