Form Builder Error - Please Match The Requested Format-

I've added a text field with nothing special and when I test it out and fill it in, I get this error "Please match the requested format"
What does that mean?

Accepted AnswerIt looks like you deleted the whole page and then duplicated the contact us page. I will recreate the page and let you know. I've recreated the form several times with the same result. Why would it give that error?

Username and password do not match or you do not have an account yet.

Accepted AnswerYes I did becuase I don't know what you're doing and you deleted one of my pages not just the form.
