Upgraded to Joomla 4 and Formbuilder no longer works

I just upgraded my site to J4 and am having the forms no longer work on the site https://www.mclawgroup.net/appointment

I tried it and sent a test email using your form. Please check that if you get it. I found no issues while filling up the form.
