Which template you are using, please? In some designs contact addon being overridden by template. That's why you do not have access to Checkbox/Phone option or/and you do not see it on front-end.

To solve it: Manually delete (!) addon folder (ajax_contact) from template, then SPPB will use original core solution, current version.

Typical path: templates\shaper_NAME\sppagebuilder\addons\ajax_contact

I am using Qubic and Onepage, if I delete the the folder ajax_contact, the contact form duplicates with the field lines hidden.
