How To Remove This Garbage ?

When I open Media Galery there is tons of crap sitting on the page. Images that I havn't added. Actuially (as you can see on the screenshot) these are not even images but a placeholders.
Why is this trash showing up there ? How can I remove it ? I tired to delete all of them once (some time gao), but they reappearing
