Form Builder Not Sending Mail

Hi,
The form builder on page builder is not sending mail, and does not indicate if the action is successful or has failed, it simply resets after clicking send. I have tried various suggestions on the forum, but to no avail. Kindly assist. The credentials are in the hidden content
