Image Select For Using In Forms

I would like to create a form like this: https://www.stablemicrosystems.com/forms/request-a-brochure/ where the user can select what they want selecting just an image and then filling a real form with their contact data.
I've being looking around and cannot imagine how to do it inside SP Page Builder neither Joomla...
Would it be possible?

By default is NOT possible, you have to find different Form component. But it won't be easy to find that it has that feature, quite unquie. Maybe ask on forum.joomla.org

Thinking about this I guess any shopping cart system could do the job. Price for each item should be 0 but that way user can pick up what info they want to receive.
I've being checking EasyShop but price is so expensive for this. I have used Hikashop in the past but I see there is no way to use a module inside a SP page, neither as a frame?
What do you think?
