SP Page Builder Form Builder Fields Not Present In Delivered Message

On my website https://artsgourmet.org/, I am trying to use the SP Page Builder Form Builder to make a form intended for application of user privileges.
I've created the form with 8 fields on a Poster Registration page and sent the form successfully but the received email contains none of the form content.
I realize that I haven't provided a subject in the body HTML code and I've deleted the subject field but am I expected my test transmission to contain all the new fields in the form.
