This is not a known issue, and based on your description, it is most likely related to caching or optimization rather than the menu itself.
Since unpublishing/republishing and clearing cache temporarily fixes it, we recommend checking the following:
- Disable Joomla cache and test
- Clear all caches (Joomla + browser)
- Check Helix template settings for CSS/JS optimization or caching and disable them temporarily
- If your server uses caching (LiteSpeed, Cloudflare, etc.), purge or disable it for testing
Also, please ensure:
- Menu items are assigned correctly to the menu module
- No access level or language settings are affecting visibility
