Rsform Appears Twice On Pages

Hi,
for some reason my forms on all form pages under Contact appear 2 times. I have assigned them to module position appt on different pages, but I only call each form once. It happens on 3 forms under the following menus: Get in Touch Book an Appointment Request a Quote
I have a screen cast for one, but it is the same problem on all 3 pages.

For Forms (created in RSForms Pro) used in Helix Ultimate use only one of those, not older (!)
why? Becuase forms looks better and .... ;]

You made a very basic mistake. SPPB and template code is/was 100% ok. If you decided to publish module inside SPPB - you cannot use for this module this same module position as you have in Template Layout. Otherwise module will be displayed twice as you had.
Use module addon OR module position for selected module. So simple rule.
