formbuilder from layoutbundle energy don´t work

Hello i use the formbuilder from the layoutbundle energy (at the bottom of the page for call back service) in my helix template, it only start loading but send no mail.

It looks like the issue is in the page template Energy. Please use form builder addon instead of the page template.
