Sorry for the inconvenience. Please check again now.
It looks like you were using the variable {{email}} in the email header (https://prnt.sc/i4SiX57RU6-_), but that variable wasn't available in your form. You have a field named e-mail (https://prnt.sc/MR8QOHCIlM1E). I've updated the email header to use the e-mail field instead, which should resolve the problem.
