Sp Page Builder Form Builder Not Working

I am trying to get the contact form on several sites on my servers to work and they won't send the form. i have reviewed countless posts with the same question and they have not resolved the matter. It is not a server issue because i have it installed on my own website and the form there works great with no issues. i tried using the same email that is configured for the domain (my own site i used the same email in all locations) and on this site it won't work. i also tried setting up a second email specific for the website ([email protected]) with no results. i can send test emails using the php mailer no problem. the form also doesn't spit back a success or failed message.
i know my server configuration supports this as it works on other sites also built with sp page builder. there are no extras installed other than JCE. i tried to disable JCE and see if disabling the extension made a different and it did not. I am using the Finion Template. Other than installing JCE, this is an out of the box website.
Can you please help?

Accepted AnswerFYI I also tried changing the captcha to the math question just to make sure it wasn't that and still no results. It's my own server so i can easily verify if the mail came in.
I also swapped the contact page form for the contact form addon but the form builder is still in the bottoms of the corporate and personal tax screens. i would really like those to work since they are my call to action.

Accepted AnswerI have tested both Contact Form and Form Builder addons on the Contact page. On Form Builder addon, I have removed email variable. Now it works. You can check the page.
