Contact form via Frombuilder not send

Hello,

On https://mooittegelsensainitair.trainingjoomla.nl/contact a made a contact form.

If fill in the form and try to send it is not working. It is not sending.

In de console i get the messe:

Uncaught SyntaxError: Unexpected token < in JSON at position 0

Why isn't it sending? What did i do wrong?

No problem. Just wait. As far as I see the error, it seems that $name is not defined in /site.php

It works fine in another host. I don't think the $name is not issue.

On the moment I sent, the icon on the sent-button is active (it's turning). In the ocnsole i see the following error:
