Form Builder Setting Problem

Greetings , I have a problem with the SP builder addon (Form Builder),
I had finished with the settings of Recipient Email , web server and the form items that I needed. Although the email had been sent by form builder and Recipient Email also received , the email content beside knowing the sender email , other information was lost . Please provide technical support regarding this issue . we appreciate your help thank you

PS: The following attachment (pictures) was the process of the testing form builder , I hope it might help get a good understanding of this issue.

Dear Khan , thank you for the assist ,

i had tried and only works on the first email which is sent

then , the problem become the form builder no longer can sent the test message to our email

although form builder message from the website said had sent success , but our email box didn't received any of it after the first success email .

i had re-check the email template selection was still our email-box.

please help us to assist this new problem
