It looks like there is a bug. It works in SPPB pages but not in modules. I have forwarded the issue to the developer team. They will check and fix it.

Apology for the inconvenience. The issue Key Not Found only appears when opt-in form is used in pop-up. I have demonstrated this bug to the developer team. It will be fixed in the next release of SP Page Builder 5. I would suggest you to move to SP Page Builder 5. After the migration, we will help you with the new issues.
