I have fixed this Form Builder as well. I would like to guide you so that you can resolve such issues on your own. You need to fix the variable names to correctly retrieve user-submitted data.

For each form item, you must set a Field Name. Then, use the corresponding variable name in the Email Body to ensure you receive the data when a user fills out the form.

When naming variables:

1. Do not use uppercase letters, capital letters, or spaces.
2. Use only lowercase letters.

Only then will the variables function correctly to fetch the data.

Please check the screencast I recorded from your site. I have shared it in the hidden content to protect your privacy. I have demonstrated only one variable example to keep the video concise. However, I have applied the same approach to all form items and updated the Email Body accordingly.
