Forms That Disappear

Hello,
We've activated the default Captcha feature on our 2 forms and since, the 2 forms are no more visible on frontend...(well, only the buttons the send the data are displayed).
We've desactivated the 2 Captcha, cleaned the caches, rebuild the 2 menus items: no chance. What are we missing here? Can you help, please?

Accepted AnswerThe form builder addon was override inside the template directory. I just renamed the site.php to xsite.php.
