FORM BUILDER - UNABLE TO SUBMIT

I have Created a form using Form Builder. When I hit the submit button I get an unable to send error message.

I have tried it with CaPTCHA and without CaPTCHA, it does not work.

I have added a Test page and it works. Please check the addon settings and use your email address in recepient email box.
