Wayne Corp Template - Opt-in Form And Acymailing Not Working / Sp Page Builder Pro

i have sp page builder pro and using the wayne corp template. i am trying to use the newsletter subscription via the optin form with it set to acymailing. I have acymailing installed, and i have created the newsletters list and set it to automatically subscribe to this. When i try to use the optin form i get either undefined (see contact page) or Some problem occurred, please try again.(at the bottom of the home page)
how can i resolve this issue?
(I am aware of an issue with the SSL certificate and working on that. It was just issued and something is wrong)
