Form Builder Not Sending Information

I cannot get the Form Bulder on a page to send the informatin. Are there any settng for this? Is there documentation for ths particulr add on? I can't find it.

You are using Convert Forms on that page. Please share your administrator access here to check Form Builder issue. Use the Hidden Content box to share the credentials. Make sure that you have a full site backup with you before sharing.
