Please check the form builder settings. Use your Global Configuration -> Server -> Mail Settings -> From Email box email address in the form builder From Email options. Then use the exact variable name (field name) in the Email Template. Do not use any variable name in the email template that does not exist. Then the form will work. Please check the screenshots attached in the hidden content box.

Yes, because in the Email Template settings you have used this variable {{subject}} but actually it has no value from your items. In your form builder items, you have not used it. If you use this field name subject in any of your form builder item, then you will get that value.
