When a user fills out a form, the email sends correctly but adds an IP address in the subject line INSTEAD of the site name. ie: Form subject title | email address | 200.1.1.1 (Site IP address instead of site name)

Is there any way to correct this. I feel like it is simple and I am just overlooking something.
