Issue with SP Page Builder Form Builder Addon

Hi,
The sp page builder Form Builder addon does not send emails whereas the Test email in the joomla system configuration works fine. The contact form does not notify any success or failure message when we click on the button to send information. All fields including the mandatory ones as the Recipient email address are properly filled in with valid email address.
Could you please investigate this issue quickly?
By the way, other posts report similar issue with the Contact form.

Hi,
I shared my access through the hidden content.
Thanks in advance for your assistance.
The form builder which does not send email is accessible through:
Components > SP Page Builder > Pages > La Tribu des Petits > section "Inscription"
The form builder is hidden. And as a workaround, I added a simple contact form just above. This one is working fine.

By the way, I have a second issue since I migrated my website to Joomla 4. It is about the menu "Off canvas menu La tribu". It is displayed but the menu does not appear on laptop nor mobile. If you have any tip to fix it, please let me know.

Apology for the delay. I have changed in the subject options. I have moved the {{email}} variable to the end of the subject. Please check your form builder now.
