They would click on the check box.
Is this the correct way to place it for the email body.

If you want to use Yes & No then you should select the radio field instead of checkbox. Thanks
I figured it out. The email address must be web based. I can't send to a personnel email address. Is there a way that can be fixed? Can I add more than one email address? To receive the information.
