AcyMailing Opt-in Custom Fields

I want to use opt-in in an SP Page, BUT with all my custom fields for the subscription (company name, phone number, etc). Can I do it any mode or not? I know, with Joomla modul it can be working, but I would like to use SP Page Builder Opt-In addon. How?
