Is there a method or process to include additional demograpic fields in the Opt-in form. I cannot see to figure out how to add anything beyond the default name and email address.

I would like to be able to add lastname, age, address, and phone number to the list of required fields without needing to revert to the embedded mailchimp form. I like the agreement checkbox the opt-in form offers.
