Error 414 - Form Builder

At the bottom side of this page, you will find a contact form. When you click this contact form, it does not redirect, but rather gives you an error 414.
What is strange is that this form is in a module, and the exact same form is also used on this page where it works 100% without any errors: https://berchtesgaden.co.za/hunting/biltong
Why can this be? The one page gives an error and the other page does not - exactly the same form inside a module.

Thanks for contacting us. Please try to load the addon inside the page instead of module. Cause, Form builder is not ready to use inside the module.
I have checked form builder inside the page and it works fine.

Sorry but that is not true. As I stated in my original question. The form is in a module. That module is used on two pages. One works, and the other does not. Please check again. The same form, inside the same module, is used on these pages:

- Not Working: https://berchtesgaden.co.za/waterberg-hunting#contact
